Abstract
Although there are numerous diagnostic means of liver disease, none of them are completely reliable or technically feasible. Splenoportography has serious limitations, frequent contraindications and often disappoints to visualize space-occupying lesions. Scintillation scanning of the liver is not only performed in a few centers and requires large and expensive facilities, but also imcompetent to show metastases smaller than 4 centimeters.
Umbilical hepatoportography is a relatively new technique in the field of hepatology. Its simpleness, easiness and accuracy are enough to overcome those defects. In addition to this, developing patency of the umbilical vein might be used in umbilico-systemic shunt for portal hypertension and in local adjunct chemotherapy for malignant neoplasm of the liver.
In view of these points, here we report 11 cases of succestful umbilical hepatoportographies done in the department of general surgery, Kyungpook University Hospital.
Primary carcinoma of the liver is relatively common in Korea comparing with the incidence in caucasian and easily identifiable on umbilical hepatoportography. We had one successful right hepatic lobectomy diagnosed by this technique. Four more cases of primary hepatoma were diagnosed also by this technique and proved by needle biopsy, but they were too extensive to be attacked surgically. Liver metastases from gastric carcinoma in 2 cases were diagonosed by this technique. Liver cirrhosis was studied in 5 cases. Two had umbilico-saphenous shunt done and one had only splenectomy performed.
